    Good afternoon, how are you? Please check our account, it has been suspended, we use a templade soon after suspended, I authorize any modification and I accept the terms of use, thank you.

    It's the FTP data field that's causing our system to auto suspend your account. We do not allow forms to collect username and a password. We consider it as phishing which is against our Terms of Use. You can read our terms here: http://www.jotform.com/terms/.

    I have reactivated the account, but I have deleted the said fields. Please refrain from adding them again. You may consider asking the FTP credential using another medium, maybe email or phone.
