I checked both of your most recent forms and did not see that field present. If this still needs adjusting, let us know which form you are having trouble with and we will take a look. If you want all current form fields with complete labels included in the notification email, you can simply delete the current notification email and create a new one:

Your email notification was customized that's why it failed to update the field labels in the template.

Many thanks however, even when you still select 'yes' to this question (Did we resolve your query during your recent contact with us?) when I receive the results via email I still this https://www.screencast.com/t/CPyGqK1X. This questions and answer should only appear in the email results if the answer of 'no' is selected. If the answer of 'yes' is selected then I do not want to question or empty text box to appear in the email. Is this possible?

Yes, that's supposed to be how email works by default. It won't show empty fields.
When I check your email under advanced settings, this feature was turned off.
We suggest setting Yes on "Hide Empty Fields" in your email settings. This should exclude the empty fields.
