My colleagues and I use a form as a checklist. Recently, the form stopped functioning normally. When I fill out the form, once, our email account registered with this form receives four blank submissions. It does not receive the information that I entered into the form.Page URL:

I did a test submission in your form and the data is showing in email logs:
And the data is not showing in previous emails. Also, none of your fields are required so the form can be submitted without filling any field.
Could you please check if the data is showing in the email?

Yes, that test submission came through to my email just fine, and it also appears in www.jotform.com/submissions.

I have successfully tested both cases using a clone of your form that I created: https://form.jotform.com/72372767289976
I did a test submission without filling any field, and one with filling the fields, and I have also checked that your email variables are set correctly. This resulted in an empty submission followed by a correct submission.
As the test submissions worked properly, I believe that, as my colleague Nik_C mentioned, the previous case could very well be a submission without filling any fields, due to the fact that none of the fields are required.
