Form not loading on a Safari browser in an iPhone

My online form would not come up on my iPhone.  do I need to do something specific to make it mobile-friendly?

We would like to apologize for any inconvenience. However, one of my colleagues who has an iPhone 7 tested your form and the form worked perfectly. The form loaded properly and my colleague was able to switch between pages as well. May we know if, by any chance, your form is embedded on any website, please? And if it is, may we ask you to share with us the link to the website so we can make a test from there? Also, when you load the form on your iPhone, does it show any specific error or warning message?
