How do I see name and email id in the submission form?

    I am not sure if I understand your query correctly. I have checked your JotForm http://www.jotform.co/form/72392260845863 and see that there is no email field added to the form. You may add the email address field from the field elements to the form so that it will be displayed in the submissions view or email notifications.
