    I think I realized why - we must have copied over a credit card field, and the site does not have an ssl set up.  I can remove the credit card fields asap, if you can unlock my account.  We are entering a busy ordering season, so your help would be greatly appreciated.  Thank you!

    Yes, asking for credit card information through plain text fields is not allowed on JotForm, it's required to use a payment integration in order to collect payment information, please find our available integrations here:  https://apps.jotform.com/category/payment_processing 

    I have already deleted the guilty fields from your form in order to reactivate your account, please check as it should be working now.

    Thanks for the help! I realized that late Friday, as we had switched over a domain, and then forgot to delete the fields.
