Hello Yegane.  You can take advantage of the form element 'Free text (HTML)' and place/embed media there such as a youtube video.  The free text (HTML) form element allows the form owner to create a customized content inside the form element.

Here is a thread which explains how to embed objects such as flash videos on your form using the Free Text (HTML) form element.  Unfortunately, you cannot add javascripts in it.

If you wish to send emails after a form submission, you have the option to create / modify your email alert notification and email alert autoresponder.  Perhaps this Send to a Friend user guide can give you an idea on what you wish to create.

For sending SMS, you can only take advantage of your mobile phone service provider's email to SMS gateway to send an SMS to you whenever you receive a form submission.  Here is a thread that talks about this.

If you have further questions, please do let us know.
