How can I create another form that looks like the first form I created?

Hi I've just started a second for and some of the page elements are now missing such as Header and Page Break. Can you tell me what I am doing wrong ?? !! Thanks 

When you create a new form, it will always be blank and won't have the same form fields you had setup on other forms. Basically, this means each form is its own form, with its respective options and setup.

If your aim is to create a new form that resembles the look and feel of the first form above, just clone it so you will start with an exact copy of the first form.
