It was suggested that I use a Configurable list, which makes the key entry of data nice and easy. However, the configurable list data is all stored in the same field for a submission. I need each data element (first name, gender, age, wish, size) stored separately as I need to "read" the data to import it later.
It is preferred to have a separate recorded entry for each registered member.

Unfortunately, this is not possible. The configurable list field is considered a single field and all data collected will count as a single entry. For more information about Configurable List widget, please visit this link: https://www.jotform.com/help/282-How-to-Set-Up-the-Configurable-List-Widget
If you want I can provide you another work around that will use the Show and Hide condition logic and Basic fields.
