I need a fillable form with selections menu and e-mail script

    Hi, 

    I run a transportation company that deals with transportation of live animal in Norway. There are strict rules for this type of transportation so the requirements for documentation are very strict. 

    Currently we have an old fashioned self-copy form that we write on and a 3 piece copy is made. We would like to move from this paper form to a digital form. 

    Since transportation happens from select pickup points and the government is very picky about this, I would like to have the field on the digital form to be selection based so that the driver can't make an error. There aren't that many pickup points either so I believe the amount of data within the pdf file would be minimal. 

    Now, when the driver has selected his pickup point he simply fills out all the other data, like today's date etc and then has the farmer sign the PDF with a stylus on the iPad/Tablet. When the driver arrives at his delivery point he notes the time and date for return, signs it and presses send. 

    The SEND button should then be stored on a Gdrive or onedrive server AND send an automatic copy to two specific email addresses. This goes for ALL forms. 

    Is this possible to do?

    If the driver must select his pickup point, you can add form fields like the Single Choice field or Dropdown field on your form.

    You can integrate your form with Google Drive so that submissions and submission data will be sent to your Google Drive folder. Here's the guide on How to Integrate a Form with Google Drive.

    You may also want to receive the copy of submissions via Email. Hence, creating Email Notifications and Autoresponder Email is also necessary.
