Why is my form not cloning correctly?

This is what the form currently looks like from URL https://form.jotform.us/72134463094151

and this is how it clones:

The form is cloning with the new layout, to disable the new layout, please go to your account settings page and un-check the "Enable New Form Layout" option.

Your form should clone with the old layout once this option is un-checked.
