I noticed when I create a report there is a "IP and submission ID" column but not the e-mail address of the person who replied. How can I have the e-mail address appear on my reports?

You should add another email field for the person who submits your form and mark it as required. Then in your autoresponder email notification, you change the recipient into the customer's email field.
