How to add my form to mobile app?

I have a question are use emailmeform that's the companies name. I want to use your company I do so for repairs and I sell accessories all through Squareup how can I implement and where so when a customer walks in the door I can take his device and he could fill in everything from my form how do I put it on the desktop on an app I'm a bit confused and lost my phone number is 954-681-6497 if you want to explain by phone or email if that's what you want to do that
