I have been using it just fiine, but then I couldn't log in one day and it said it was suspened...

Your loan/credit application form collects SSN and drivers' license# which are considered sensitive account personal data.
We can only request for consideration to reinstate your account if you agree to fix the form and remove those fields that have sensitive data on it.
Let us know if you agree to this.

I was curious as to how I am able to use those fields... I read in your terms that I can use it as long as its SSL secured.. "You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features"
Do I have to upgrade my account?

You do not have to upgrade your account.
Termination. You agree that Company may terminate your JotForm Service membership or suspend your access to all or part of the JotForm Service, without notice, if Company determines, in its sole and absolute discretion, that you have violated these Terms.
Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
We understand this may not be the case yet we had to act accordingly to comply with our Terms. Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.
If you want to ask for SSN and Driver License, you must provide us the following:
1. Your company's website URL
2. Your corporate email address
Please send an email to firstname.lastname@example.org using your corporate email address. Include in the subject line the following: ATT: (John_Benson) Thread 1237547 and briefly explain the purpose of your forms.
