Error. 401

why am I getting an error when I go on the site and it view form but the link I sent out to my customers is live 

I checked your forms and they all appear to be active.  Is there a specific form or method for reproducing the error?  Also let us know if the form is embedded to a web page.  From what I can tell though, your forms should be 100%.
