Is there a Checkout Widget that allows image uploads?

I am trying to create an equipment checkout form for staff. I want the staff member to choose what they want to checkout but also have an image of the item next to the text for reference. We have many events and are constantly updating and adding banners and other promotional items to our inventory. It is helpful for our staff members, especially those in our satellite offices, to see pictures of what we have in inventory. 

You can try using the "Image choices" widget I think it will meet your requirements. Below is a link to a sample form I have created.
