We have a tool that will allow you to import your PDF document to JotForm and it will be converted to PDF form in JotForm.

    Check here: https://www.jotform.com/fillable-pdf-form-creator/

But I checked your PDF document and I found it does not have input fields on it. Our tool will not be able to import it.

Unfortunately for this case, you will have to re-create the form using the Form Builder. In my assessment of the PDF document, it look like it can be easily recreated in the Form Builder.

You can use your PDF as your template/guide also while creating it in JotForm.

I hope this help. Let us know if you require further assistance.

I created a form from scratch thru create form. I need a conditional drop
    down.
If the school field equals to let's say,"tchs main". I want the route #
field to drop down certain choices. And so on. If another school is filled
different choices are to drop down.
Please advise the best route to accomplish this task.

We would like to apologize for any inconvenience, unfortunately, we do not have a feature that changes the option in a drop-down field based on another field's value or selection.

What I can only suggest you do is to add a different drop-down field for each school. Each drop-down field has its own options different from the others.
