Items on my form are dissappearing when the form gets previewed or goes live

On my latest form, you can see "select program" and "payment options" , then when you preview or publish they are not viewable....Please help!

I cloned your form and I found out that you are using Show and Hide conditions. This condition show or hide a specific field if the condition is met. There are also missing fields in the conditions.

I recommend you delete these conditions in order to fix the issue. Select the condition and then click the "Properties" icon. After that, click the "Trash" icon.
