Is this your Weebly website? I do see the published form on the website and it is working when I loaad on my browser.
If this is not the issue please provide more clearer details about it and we will check further.

I test the link on the Hamilton application website page and it was working as well. The form was loaded in the browser using the form URL name.

I will not be able to view the website page https://www.weebly.com/weebly/main.php#settings/contributor since that link is on your Weebly website editor only.
Can you please provide your Weebly website actual URL where you wanted to publish the form?
You can try also adding the form to the website using instead its iframe publish code. See if using the iframe embed code resolves the issue already.
