There are 2 ways you can create a custom name to your form link.

#2 How-to-use-Custom-Domains-with-Jotform

 

The #2 option will require you to setup the custom domain name on your side. You must have understanding of domain CNAME on how it work. 

The 1st option is very much straightforward and simple to do in the Form Builder. It changes the URL text/link your form use.
