Google analytics not tracking based on correct channel

Hi, we recently changed our sites application form to a Jotform. However since the change, Google is not tracking goal completions based on the original source channel (i.e paids/organic/direct).

Goal conversions are shown as "referral - jotform".

I have added the GA widget and added the tracking ID, but this doesn't seem to have helped.

How do I fix this?
