My account got suspended

    The form ID (72466180056557) is asking for credit card information using basic fields. That's the reason why your account was suspended. As per our terms, collecting credit card information is not allowed here in JotForm.

    We can re-activate your account if you let us delete the guilty fields or delete the form.
