I checked your form and I can see that the MailChimp integration is not configured correctly. The fields are not yet mapped in the integration. Here's a screenshot:
Please make sure that the fields in the list are mapped correctly. You may refer to this user guide: https://www.jotform.com/help/163-How-to-Integrate-with-MailChimp.

the Email on Match Your Fields..And then when i click on it the only option
is newsletter..the other boxes options are empty..And then when i click,
Add new field, the options are just the same I mentioned above
(newsletter,and the other boxes options are empty). What's wrong with my

I checked the MailChimp integration on your form (http://www.jotform.me/form/72311002234436) and I was able to replicate the issue. The only option from the dropdown is the Newsletter.
This is happening because the fields of your form have no name. Please add a name for each form fields so that it will appear in the MailChimp integration.
