Account Suspended? Not Sure why?

Unable to login to my account this morning.  Suspended.  Can you help please?

I checked your account and it looks like one of your forms requests usernames and passwords.

To protect users filling out forms, we have a blanket ban on collecting usernames or passwords.  As per our terms of use:

You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

We can remove the form and activate your account.  Let us know if you would like for us to do so and we will take care of it as soon as we can.
