I cannot login with the password that was correct as of last week. When I try to do password re-set I don't receive any email from with instructions.

    The email address (ghhm@ucsf.edu) is on our bounce list and that's the reason why you're not getting the password reset email.

    I have removed it from our bounce list. You should be able to receive the password reset email. 

    Upon checking it further, I can see that the email address (ghhm@ucsf.edu) is not associated with any JotForm account. 

    Are you referring to the account username (healinitiative) that is registered to the email address (phuoc***@ucsf.edu)? If yes, please log in to that account and if the issue persists, let us know.
