Help to fill out a form

trying to  set up a checking account for refund slection.com it will not let me put in all info and it says their  information does not match to ask FAQ

I checked their website and I did not see any embedded JotForm on it. If you have any issues with filling up their form, please contact them or the administrator of the website.
