Why has my form disappeared?

I spent quite a bit of time creating a form a week or so ago.  This evening I logged into JotForm and my account shows that I have no forms.  I did NOT delete my form.  I have the input form short cut that I saved to my desktop https://form.jotform.us/72315265084151 .  Is there any way to retrieve this form at all?  Do you have any idea how this form could have disappeared?

I have checked your account activity logs and noticed the form was deleted on August 23, 2017:
