Form appears differently in my account and my partner's account.

    Also when I log onto my account, it appears we both have different settings/drop boxes and options available - can you please explain why?

    I did check the form you have shared and it seems have been created with new form layout. Please note that you can change your form layout to "Legacy Layout" and that should solve your problem. Please check the screenshot below on how to change form layout:
