I can not login to my account

I have checked your JotForm account ahmed_asd  and see that it is suspended due to the form  http://www.myjotform.com/form/60513484714555 which is collecting user names and password along with facebook logo. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.  

In order to activate your account, it is required for you to agree with our terms of usage and delete this form from the account. Please confirm so that we can delete the guilty form and activate your account.
