Why is my company logo not populating when I receive a filled out form?

The logo you have on your form will not be included on your Email Notification template by default. You'd have to manually add it to the email body content if you wish to include it on your emails.
To do that, just navigate to SETTINGS > EMAILS > PENCIL icon on your notification > then edit the EMAIL CONTENT section of your email. From there, click the INSERT IMAGE button on the formatting toolbar to insert the logo.
