Unfortunately, I was not able to replicate the issue. I was able to open your form on my iPhone. Here's a screenshot:

Please take note that you have a long form and you have a lot of conditions. Actually, it took me 3 minutes to load the form.

    Hope that helps. Thank you.

I waited 5 minutes on my iphone and it would not open. I understand  my form is extremely long, but is there anyway to speed us the loading speed.  Secondly, my form is extremely slow on the submit page.  If I were to embed my form into a website, would this possibly make the form faster?

Mobile devices tend to be significantly weaker than desktop devices.  Mobile browsers also tend to be more limited in resources because of this.  When a form has many hundreds of fields and several hundred conditions, it is quite difficult for the browser, especially mobile browsers to process them.  This is a limitation on the system viewing the form, not on the form itself.

The same goes with the submit page.  Every field, every condition every validation needs to be checked before the form can complete the submit process.  Embedding the form to a web page would make no difference, the form would load the same.

The only way to make the form run smoother would be to reduce the size and number of conditions.
