We created an email to be sent upon completion of our form. Upon testing the form, I have discovered the email is not being sent. How do I rectify this problem?

I did a test submission to your form and I received the autoresponder properly. Here's a screenshot:
Please try it on your end and do not use the test button to the notification email or autoresponder. Try to fill up the form manually and click the Submit button.
