my account has disappeared! what happened?

    I am a paid subscriber to your service and now my account seems to have disappeared. I just went to create another form and could not sign in. I was told there was no account with my username. However I am still recieving jotform responses from already posted jotforms. Please Help! Please contact me as soon as posible. My needs are time sensitive.

    Form ID is a string of digits at the end of its URL. The ID can be seen when form is loaded in the form builder and the Preview button is clicked:
