Is there a way to add company logo to the form being submitted?

Also, is there a way to add your company logo to the form being submitted? I have added ours as an image but it doesn't appear on the form.

I am not sure if you are referring to add JotForm logo to the form? As I check your last edited form, I see that a logo is being displayed on the form.
