My form won't transfer to PDF format

I need help getting this to work.

 

What do I need to do?  It starts the process but then never finishes.

I cloned your form and have been inspecting it, I noticed you have some conditions and page breaks on it, please note that such elements are not currently supported on PDF fillable forms. 

I have removed the conditions and page breaks from my cloned form and it worked fine, I was able to download it as PDF fillable so I would recommend you to do the same on your end.

I followed your instructions and it did create the form, but there are no words on it – see attached.
Can you please help me! I don’t have time to figure this out today and need to get this to a client.

I would also recommend you to try using a different browser, Chrome and Firefox are the most recommended.

Yes, I got the same form, but if you look at the top there is no logo – just weird blank pictures.

Can the form be downloaded without the weird top. It look so unprofessional.

Could you please provide us with a bit more information? Do you need a fillable PDF for online form submission/filling, or just a PDF for printing/emailing? It should be easier for us to get a non-fillable PDF by using a browser print to PDF option.
