Several of form fields in submissions are empty.

These questions have come out on the form for Mr Steve Bakamund but not Mr Milo Astaire - same form, same link given to both patients

"Email", Everything from "Age" to "Doctors name, surgery and phone number", Also "How many times have you visited your GP"

Also Health priorities have come out as Priority 1, Priority 1 for Mr Astaire - so a Priority missing and one named incorrectly and Priority 1, Priority 2, Priority 1 for Mr Bakamund - so the last one named incorrectly

We have also had several past problems with the question "Blood Group (if known)" not appearing at all

This inconsistency is unacceptable and makes us look unprofessional

Please advise. If necessary reupload the form making sure the existing notifications are turned on as we have had problems with this before. NOT IMPRESSED WITH YOUR SERVICE. PLEASE ADVISE ASAP

I am not sure if I have understood the the issue you are having. Do you mean to say that several of your form fields are missing in submission? 

I did check the last submission you have received and it does not have "Priority 3". The issue is that several of your form fields are not required field and user can skip that question and still submit form. Please check the screenshot below:

Since several of your form field are not required field, users can skip those fields and submit form. I would suggest you to please make desired questions as required field and that should solve your problem. Once the fields are required, users will not be able to skip questions. Please check the screenshot below on how to make form fields required:
