I would like to add a checkbox field which if selected will duplicate the page

I have a requirement where I want to capture the following fields Primary address location, Billing address, mailing address, contact details, few other checkbox fields for one Factory location. I would like to have the ability where I could allow the user to click on a button or a checkbox and create the same fields above to enter the second factory location. If there 10 factory locations then they would click this button or check the box 10 times to add those location details. I saw the Configurable list widget that kind of delivers this functionality but then I am unable to list the fields vertically. So please suggest an option to meet my requirement.
