Adding my form to a category in on my website

okay were getting close, instead of a page how do you add the form to a category or product on my website. So that the customer can go to the category and the form pull up. The page is located at the bottom and I'm scared that will confuse them. 

Also so on my form some tabs have 0 and some tabs are blank and under name title it keeps being preselected and nothing is there. On Homecoming lettering it doesn't even check you have to scroll to the option. 

How do do you do a subscription as well and add to big commerce.
