Why images do not display once I download the pdf of the submission?

I have created a clone of an existing form as well as a test form, in both cases the images (including the logo at the top) do not show when the submission is downloaded as a pdf document!

What am I doing wrong?

On the Submissions page, you need to enable the "Show Headers and Texts" to fix the issue. Please check this guide: How-to-Show-Headers-and-Text-in-the-PDF.
