How can I create a file upload form?

Hello, I am trying to create an online upload form that would serve as an audition form/resume for the actors that audition for my theatre program. I would like a space where they could insert a photo of themselves, answer the questions or input the personal info I decide upon and then when it is submitted, they will all be submitted to one place where it can possibly organize data, etc. Is this even possible? I am just starting to attempt this process and I am happy to learn the program, but I want to know if it is at least possible. Thank you!

You may add a File Upload Field to your form to achieve this. You may setup the field to only accept image files, and limit the file size upon your discretion.

All submissions done on your form will be consolidated on your Submissions Page. You can view them from there or even export them to XLS, CSV or PDF format when needed.
