Some applicants are not able see the classification table added in the application form. Attached is the screenshot.
Please solve this issue at the earliest.

As I check your web page at my end, it seems to be working fine at my end. Please see the screenshot below:
Could you try opening the web page in a private/incognito window or a separate browser to check if that works? You may also check if the direct URL to the form https://form.jotform.me/71231033499453 is working fine.
