Why my form is not showing properly when viewed on phone?

Ho appena provato la versione mobile in verticale, 

però i campi escono dalla cornice...

The CSS added to your form is causing the formatting issues one mobile.  Try simply removing the CSS currently added to your form. 

The form looks nearly the same on desktop but the formatting on mobile is much better.
