How to import data for users to choose from?

I'm creating a form that will help my customer service department record notes when assisting a customer with a return. I would like to know if there is a way to import customer information (order number, item number, etc.) into the form so that when the a CS agent uses the form, they can choose the order number and item number of the customer. 

If you want to import data from an outside file/source to show on the form as field input that is based on a selected input on another field on the form, I suggest you use the Spreadsheet to Form widget. 

You will upload the file with your customer's data on the widget. The widget requires an Excel file (.xls, .xlsx, or even .csv) and it won't accept any other format.
