Hello,
I have a use case of 5,000 contacts with information about those contacts such as name, email, phone, address etc. I want to email all asking if they have any updated contact information. The email should be formatted like this:
Hello [First name],
Here is the phone number I have on file for you
[Phone Number]
If you would like to change it click here"
If possible i'd like to set up a call ASAP to discuss.

Yes, it is indeed possible to create such template and send it to your submitter, you will first need to add an email auto-responder to your form, this guide will help you with that: https://www.jotform.com/help/26-Setting-up-an-Autoresponder-Email
Once added, you could include fields from your form on the email template:

Hi Kevin, thank you. Follow up question.
When it comes times to distribute my form to users I can either email it with a link to the form or I'm assuming I can embed some HTML code on a public site. Is this correct?If sending the form via email is there any way to change the URL that the person receives? I worry we won't get a lot of clicks because our end users will not recognize the URL and not click on it.
