Custom URL attempt is "saving" but does not appear to be linked to any form.
Steps: Source code, URL, advanced iptions, custom URL, my custom name is there and saved, but this URL seems to be broken link.
What am I doing wrong?

I will answer myself:
there was a space in the custom portion Cardiology Invitation, which became Cardiology_Invitation in the custom URL.
For whatever reason, that link did not work.
I tried just Cardiology, and voila, it worked.

That's the thing about coding in general: Every semicolon has to be in place, no comma should be missing... all sorts of seemingly slight mistakes can throw everything out of whack. For instance, a common mistake many people who can't get images displayed on their online webpages (even though they display just fine in their browsers locally) is to have the extensions of those images in ALL CAPS. It's usually not their fault. It's just that some image editing applications do that.
JotfForm is "Awesome software." I totally agree! :)
