Unable to create PDF version of form

Dear Support team,

When I click on the "create PDF" option so that I can print out the form created, it says working with a spinning wheel but it never creates the form.

I tried using different browsers but it does not seem to make a difference.

Would you have any suggestions or workarounds on how to fix this?

I tried cloning this one, and it took around 30-45 seconds to get finished and downloaded.

Thanks for looking at this form me.

That is indeed the correct form.

Unfortunately when i click on the Create PDF form it keep saying is "Working" no matter how long it leave it for (I left it for 20 minutes before giving up).

Would you have any suggestion, or alternatively would you be able to send me the PDF version of the form?

I have cloned your form on your account. Can you please try that cloned version instead.
