Is it possible to have a variable number of fields on the form?

Hi there, new user here.  I am wanting to know if there is a way to setup a form with a variable amount of repeated fields.  What I am trying to do is setup a registration form for a family.  The family may have multiple players between 1 and 5.

The only thing I need to capture on each player is First Name, Last Name and Date of Birth.

After entering one player can there be a button or something allowing them to add more players to the form or do I need to setup fields for up to 5 players?
