Question about a Form I want to Make

    Is it possible to make a form that does the following for my service based business that travels to people's locations:

     

    Collects Contact Info

    Allows us to make notes of the issues the customer has

    Then a box we can check off for the type of service needed

    Then A Signature box that "shows" the list of issues the customer describes (this way when person is signing they see at the same screen what they are agreeing to issue wise)

    Then a box my employee can fill out about what issues we fixed and addressed

    Then a signature box that has a option for customer to type name with date/time to confirm work was completed and they are satisfied
