I cannot open the attached .jpg file

The form you provided was created in the account ApartofMelb. You are not able to download the image because of the Privacy settings of that account. 'Require log-in o view uploaded files' option is checked in the account settings page.
If you are a sub account user of that account you have to log in to your account to be able to download uploaded files.
You can also contact the owner of the form and tell him/her do disable mentioned option so that you can download the image that was uploaded.
