Why is my login suspended and how can I get it active again?

    I was checking to see if my created form still worked, and I got to know that the form is not available. Upon further check, I realized that my account has been suspended. I have not been provided any reasons for the same on my email.

    I would like to reactivate my account and form as soon as possible.

    I have checked your Jotform account saurabhrdeshpande and see that it is suspended due to the form is collecting sensitive information like credit card numbers and bank account information using plain text fields which is not allowed with JotForm. Unfortunately, it is not allowed to collect credit card information on plain text fields using JotForm. Such forms are considered as phishing forms and associated accounts will be automatically suspended. Please refer to our terms of usage for more information on phishing. If you need to receive payments, it is required to use one of the payment methods available in the form builder. These payment methods comply with the PCI/DSS security standards to process the payments securely.  

    It is required for you to agree to our terms of usage and agree to delete the guilty fields on the form in order to activate your account. Please confirm so that we can remove the guilty fields on the form and activate the account.

    Ok, I removed the offending fields from your form and reactivated your account.
